单纯性肝囊肿不同外科治疗方法的探讨

目的探讨单纯性肝囊肿合理的外科治疗方式。方法对51例单纯性肝囊肿患者分别行剖腹开窗术、腹腔镜下开窗术及超声引导下囊肿穿刺抽液术的临床资料进行回顾性分析。结果腹腔镜下开窗术及剖腹开窗术术后囊肿复发率明显低于超声引导下囊肿穿刺抽液术,为0复发率,其差别有显著的统计学意义,p<0.01。腹腔镜下开窗术与剖腹开窗术相比,其手术时间、住院时间、手术并发症又明显少于剖腹开窗术,其差别有统计学意义,p<0.05。结论剖腹行肝囊肿开窗术,创伤大,恢复慢,并发症多。腹腔镜肝囊肿开窗术具有操作简单、创伤小、痛苦轻、患者康复快、住院时间短、复发率低等优点。单纯性肝囊肿抽液术虽然创伤轻、痛苦小,但仅能暂时缓解症状,复发率高。

EXPLORATION OF VARIOUS SURGICAL TREATMENTS FOR SIMPLE CYST OF LIVER

Objective to explore reasonable surgical treatments for simple liver cyst. Method the clinical data of 51 cases of simple hepatic cyst, in terms of Caesarean fenestration operation, hepatic cyst fenestration with laparoscope and ultrasound - guided cyst puncture fluid pump surgery, were respectively and retrospectively analyzed. Results The recurrence rate of the cyst both after Caesarean fenestration operation and the hepatic cyst fenestration with laparoscope, which were 0, were evidently lower than after undergoing ultrasound - guided cyst puncture fluid pump surgery. Such a difference showed a statistical significance （p 〈 0. 01 ）. The hepatic cyst fenestration with laparoscope, compared with Caesarean fenestration operation, needed a longer operation duration and hospitalization, but gets less complications. And the statistical significance was showed from the difference （p 〈 0. 05 ）. Conclusion patients suffering hepatic cyst, after undergoing Caesarean fenestration operation, might get more trauma and recover slowly with more complications. While hepatic cyst fenestration with laparoscope, with the advantages of simple operation, less pain, short hospitalization time and low recurrence rate, may help the patients recover fast. The ultrasound - guided eyst puncture fluid pump surgery, though makes little trauma and pain, with high recurrence rate, can only be used for temporary symptom relief.
