经皮肝穿胆道内外引流术治疗黄疸的初步评价

目的]评价经皮肝穿胆道内外引流术(PTCD)治疗中医黄疸的临床疗效。方法]55例恶性梗阻性黄疸患者按照中医辨证分成阳黄、阴黄两组,两组患者均行经皮肝穿胆道内外引流术治疗,观察黄疸改善情况及并发症情况。结果]阳黄与阴黄两组术后1月、术后2月总胆红素、直接胆红素与术前相比明显降低;术后3月总胆红素、直接胆红素两组间比较有显著性差异(P<0.05),并发症发生率分别为9.6%,16.7%,引流管堵塞率分别为3.2%,16.7%。结论]经皮肝穿胆道内外引流术可短期内明显改善不同证型中医黄疸的临床症状,安全有效,其中治疗阳黄患者疗效更佳。

Prospective Trial of Percutaneous Transhepatic Cholangial Drainage in Treatment of Jaundice of TCM

Objective] To evaluate the clinical value of percutaneous transhepatic cholangial drainage(PTCD) for the different syndromes of jaundice of TCM.Method]55 patients with malignant obstructive jaundice were put into two groups yin-yellow and yang-yellow by differentiation of TCM,who were treated by PTCD and valuated the effect and the complications.Result]The levels of serum totle bilirubin,the direct bilirubin of all the patients decreased significantly after procedure in 2 months.There were obviously difference between the two groups in the third month after procedure(P<0.05).Complication rate was 9.6%,16.7%,and drainage tubes blocked rate was 3.2%,16.7%.Conclusion]PTCD can improve the short term clinical effect of different syndromes of jaundice of TCM significantly,safe and effective,including better treatment efficacy in patients with yang-yellow syndrome.
