眶壁颌面部复合性骨折的早期手术治疗

摘    要:目的观察与评价眼眶颌面部复合性骨折的早期手术治疗的疗效。方法本组16例眶壁颌面部骨折,13例在伤后24小时以内,面部肿胀发生以前手术:3例在伤后4~7天,肿胀消退后即刻手术。用钛板作眶缘及颌骨内固定,重建眶缘的完整性,然后用硅胶片及钛网作眶壁修补。结果随访3月~2年,本组16例(17眼)面部畸形均得到良好改善,复视及眼球运动障碍恢复正常,咬合错乱及张口受限恢复正常,对视力无影响。未出现对钛板、钛网及硅胶板排斥反应。结论眼眶颌面部复合性骨折的早期手术,容易达到解剖复位,可减少眶内容因嵌顿而发生的缺血、坏死、纤维化和萎缩的几率,提高疗效。

Abstract:Objective To evaluate the effect of early operation treating orbital and maxillofacial complex fractures.Methods 13 patients with orbital and maxillofacial complex fractures were operated on in 24 hours after injury and before swelling,3 patients were operated on in 4-7 days.We applied the taitanic plates to orbital and maxillofacial fixation and reconstruction the orbital completeness and after then the silicon gel sheet,taitanic net to repairing orbital injury.Results After 3-24 months follow-up,the face malformation was well improved,double vision,the dyscinesia of eyeball,the limitation of mouth opening and occlusal disturbance were restored to normal.There were no influence on vision and no rejection on the object which was implanted.Conclusion The operation way is easier to reach anatomical reduction,cut down the happening of ischemia,necrosis,fibrogenesis and atrophy,from which orbit was incarcerated.It raises the effect of the operation.
