老年下肢骨折患者腰硬联合麻醉的临床应用分析

目的对老年下肢骨折患者腰硬联合麻醉的临床效果进行观察和分析。方法选取2009年10月～2011年10月在本院进行下肢骨折手术治疗患者204例,随机分为两组,甲组患者100例,在单纯硬膜外麻醉方法进行手术治疗;乙组患者104例,在腰硬联合麻醉方法下进行手术治疗,对比两组患者的麻醉效果。结果乙组患者的术中生命体征对比甲组患者,差异无统计学意义（P〉0.05）。乙组患者麻醉起效时间显著短于甲组,差异具有统计学意义（P〈0.05）。乙组患者术中硬膜外麻醉用药总量显著低于甲组患者,差异具有统计学意义（P〈0.05）。两组患者均未发生因麻醉操作不当导致严重不良后果发生。结论对老年下肢骨折患者采用腰硬联合的麻醉方法具有麻醉效果好、麻醉起效时间短、硬膜外腔麻醉用药总量少的优点,安全性较好,适宜广泛应用。

Clinical analysis of combined spinal and epidural anesthesia used in elderly patients with lower limb fracture

Objective To observe and analyze the effect of combined spinal and epidural anesthesia used in elderly patients with lower limb fracture. Methods Two hundred and four cases of patients with lower limb fracture, who un- derwent surgical therapy in our hospital from October 2009 to October 2011 were randomly divided into two groups. 100 cases in group A were given surgical therapy under only epidural anesthesia, 104 cases in group B were per- formed under combined spinal and epidural anesthesia, and the anesthetic effect of patients in two groups were com- pared. Results There was no statistical significance （P 〉 0.05） in intraoperative vital signs between two groups. The onset time of anesthesia in group B was significantly shorter than that in group A, the difference was statistically sig- nificant （P 〈 0.05）. The total dosage of epidural anesthesia in group B patients was significantly lower than that of group A, the difference was statistically significant （P 〈 0.05）. There was no serious adverse consequence caused by misoperation of anesthesia in two groups. Conclusion It has good anesthetic effect, short anesthesia onset time, few to- tal dosage and safety that combined spinal and epidural anesthesia used in elderly patients with lower limb fracture. It is suitable for wide application.
