三种评估腭咽功能方法的对比研究

目的：探讨一种在临床上实用、简单的评估腭咽功能的方法。方法：运用主观语音评估、头颅侧位x线咽腔造影及鼻咽纤维镜对16例非综合征性腭裂患者术后的腭咽功能进行评估，分析三种检测方法之间的一致性。结果：62．5％（10／16）的患者经三种方法检测后腭咽功能评估结果相同。相对于鼻咽纤维镜检查，语音评估与头颅侧位X线咽腔造影结果的一致性分别为75％和82％，Kappa值分别为0．66和0．73（P〈0．01）。结论：主观语音评估或头颅侧位X线咽腔造影可作为评估腭咽功能较简单，精确的方法。

Comparative study of three methods on assessing velopharyngeal function

Objective:To explore a simple and pratical method on assessing velopharyngeal function.Method:16 consecutive patients were investigated with nasoendoscopy,lateral cephalometrics and perceptual speech analysis.Percent agreement and kappa calculations were used to include degree of reliability based on probability among three judegments.Result:10 of the 16 patients(62.5%)were recommended the same action regardless of the amount of presented information.Percent agreements of lateral cephalometrics and perceptual speech analysis was 0.66 and 0.73 respectively,compared with nasoendoscopy.Conclusion:Perceptual speech analysis or lateral cephalometrics is recommended to be the minimal standard for velopharyngeal assessment,and nasoendoscopy could be added on request.
