上下泪小管断裂鼻侧断端的寻找方法及疗效观察

目的探讨上、下泪小管同时断裂鼻侧断端的寻找方法及吻合术后疗效观察。方法对门诊收治的21例(21只眼)上、下泪小管同时断裂的患者在手术显微镜直视下断端处滴水,嘱患者捏鼻用力憋气,通过发现鼻侧断端有气泡冒出从而寻找到泪小管断端处,用6号猪尾巴探针导入软性医用硅胶管做为泪道支撑物,同时吻合上、下泪小管,硅胶管留置3个月。结果 21例(21只眼)全部找到断端吻合,术后3个月取出留置硅胶管,18例无溢泪,泪道冲洗通畅,占85.71%;3例轻度溢泪,泪道冲洗通,占14.29%。结论在手术显微镜直视下嘱患者捏鼻用力憋气,通过发现鼻侧断端有气泡冒出从而寻找到泪小管断端,以软性硅胶管做为泪道支撑物,同时吻合上、下泪小管的方法操作方便,效果良好,适合临床推广。

Clinical observation and method of locating the nasal ends of both upper and lower lacerated lacrimal canaliculi anastomosis
