武汉市结核病患者治疗费用及其影响因素分析

目的:了解结核病患者诊疗费用情况,分析影响肺结核治疗费用的主要因素。方法:对武汉市硚口区和江汉区23家社区卫生服务中心所有在治结核病患者进行问卷调查,采用χ2检验和广义线性回归的方法进行统计分析。结果:结核病患者确诊后治疗总费用平均为10400元。单因素方差分析显示,低收入患者确诊结核病的住院费用以及总费用均显著低于非低收入人群(F=5.390,P=0.023;F=4.332,P=0.040)。广义线性回归模型分析显示,结核病患者住院次数越多、年龄越小、文化程度越低,诊疗费用越高;城市低收入家庭诊疗费用较低。结论:采取综合管理措施,减少住院次数及住院日,重点督导男性青壮年患者,加强对低收入家庭患者费用减免力度,确保诊疗规范、及时、有效,是有效降低诊疗费用的重要途径。

The Analysis of Treatment Cost and Its Influencing Factors of Tuberculosis Patients in Communities,Wuhan

Objectives:To investigate the treatment cost of tuberculosis and analyze its main influencing factors.Methods: All the tuberculosis patients in 24 communities of Qiaokou and Jianghan District of Wuhan City were investigated with a questionnaire.χ2 test and multivariate regression analysis methods were used.Results: The total cost of TB treatment after diagnosed was 10400 Yuan.Single factor analysis of variance found that hospitalization cost and the total cost of low-income patients was significantly lower(F=5.390,P=0.023;F=4.332,P=0.040).Multivariate regression analysis showed that TB patients with more TB hospitalization frequency,much younger,lower education level had higher treatment cost,the cost of low-income families was lower.Conclusion: Comprehensive measures should be taken.Reducing the frequency of hospitalization and impatient days,key direction young male patients,strengthening low-income patients’ fee deration and diagnosing normally,timely and effectively are the important ways to decrease the treatment cost.
