丙种球蛋白对儿童难治性肺炎支原体肺炎疗效的Meta分析

摘    要:[摘要]目的:系统评价丙种球蛋白对儿童难治性肺炎支原体肺炎(RMPP)的疗效。方法:检索维普、万方数据库、中国知网、中国生物医学文献数据库,结合纳入标准和排除标准严格筛选文献,采用改良Jadad量表评估纳入文献的质量,以总有效率、体温恢复时间、咳嗽缓解时间、肺部啰音消失时间等为主要疗效指标,应用RevMan 5.3软件对纳入的随机对照试验进行Meta分析。结果:共纳入7篇文献,包括580例患儿,其中对照组277例,试验组303例。Meta分析结果显示,丙种球蛋白可缩短RMPP患儿的体温恢复时间(MD=-1.60,95%CI -2.09~1.12,P<0.01)、咳嗽缓解时间(MD=-1.73,95%CI -1.85~-1.60,P<0.01)及肺部啰音消失时间(MD=-0.72,95%CI -0.93~-0.51,P<0.01),提高治疗总有效率(OR=5.17,95%CI 2.45~10.90,P<0.01)。结论:丙种球蛋白对儿童RMPP的疗效显著。

Abstract:[Abstract] Objective: To systematically evaluate the efficacy of immunoglobulin for refractory Mycoplasma pneumoniae pneumonia (RMPP) in children. Methods: VIP, Wanfang database, CNKI and CBM database were retrieved, literature were screened strictly according to the inclusion criteria and exclusion criteria, and the quality of the included literature were evaluated by the modified Jadad scale. The total effective rate, recovery time of body temperature, relief time of cough and disappearance time of lung rale were set as the main efficacy indicators. The Meta-analysis of the included randomized controlled trials was performed with RevMan 5.3 software. Results: Seven literature and 580 samples were enrolled, including 277 cases in the control group and 303 cases in the experimental group. Meta-analysis showed that immunoglobulin could shorten the recovery time of body temperature (MD=-1.60, 95%CI -2.09~1.12, P<0.01), relieve time of cough (MD=-1.73, 95%CI -1.85~-1.60, P<0.01), and improve the total effective rate (OR=5.17, 95%CI 2.45~10.90, P<0.01) in RMPP children. Conclusion: The efficacy of immunoglobulin for RMPP children is remarkable.
