二维超声征象多参数综合分析对乳腺良恶性肿瘤鉴别诊断的意义

摘    要:目的 探讨二维超声征象多参数综合分析对乳腺良恶性肿瘤鉴别诊断的意义。方法 应用高频二维超声对乳腺恶性肿瘤观察以下指标:病灶形态,病灶边界,病灶周围,病灶内部,病灶后方及有无微小钙化灶,同时选择乳腺良性肿瘤作对照组。两组病例超声诊断与手术病理诊断结果进行对照。结果 乳腺癌组38个病灶(71.7%)大体形态不规则,41个病灶(77.4%)边界有毛刺或锯齿状,43个病灶(81.1%)周围无包膜或壁,40个病灶(75.5%)内部回声不均匀,9个病灶(17.0%)后方回声衰减,17个病灶(32.0%)内部有微小钙化灶。超声诊断乳腺癌与手术病理诊断符合率为95.3%(41/43例)。结论 二维超声征象多参数综合分析对大多数乳腺恶性肿瘤有助于明确诊断,但对微小乳腺癌的诊断仍有局限性。

Abstract:Objective To investigate the differential diagnosis value between breast carcinoma and benign tumor by comprehensive analysis of multiple parameters in 2D high frequence ultrasonography (HFU) features. Methods The data was surveyed by 2D HFU, such as tumor shape, boundary, internal echo, attenuation and small calcification. Benign tumors of breast were the control group. The ultrasonic diagnosis of these two groups had been compared with pathology diagnosis. Results Among the breast carcinoma, 38 tumors (71.7%) were irregular shaped, 41 tumors (77.4%) boundary was angular, 43 tumors (81.1%) had no capsule or wall, 40 tumors (75.5%) had heterogeneity internal echo, 9 tumors showed echo attenuation, and small calcification was found in 17 tumors (32.0%)interior. The ultrasonographic diagnosis of breast carcinoma correlated with pathology diagnosis, ultrasonographic coincidence rate was 95.3%(41/43). Conclusion The comprehensive analysis of multiple parameters by 2D HFU features is helpful to definite diagnosis in most breast carcinoma, but it is limited in diagnosis of small breast carcinoma.
