后路经关节突人路微创减压植骨内固定术治疗胸腰段爆裂性骨折研究

[目的]通过对比研究观察后路经关节突入路微创减压植骨内固定术患者的手术疗效。[方法]2007年6月～2009年6月,选择胸腰段外伤性爆裂性骨折患者共62例,男47例,女15例,年龄22～41岁。按自愿的方式随机分为两组,常规后路全椎板减压植骨融合组(N=32例):伤椎临近节段椎弓根螺钉置入复位后路全椎板减压后外侧植骨融合术;经关节突入路微创减压植骨内固定组(N=30例):伤椎及临近上下节段椎弓根螺钉置入复位经关节突入路微创减压植骨融合内固定。[结果]所有患者均完成1年随访,主要观察指标为术后随访时胸腰段骨折术后矫正率、患者术后腰痛JOA评分,通过对比研究分析可见微创手术组效果明显优于常规手术组。[结论]后路经关节突入路微创减压植骨内固定术对于胸腰段爆裂性骨折有很好的治疗效果,用微创减压植骨内固定技术可以在减少手术创伤的同时达到较好的稳定脊柱的效果,此方面的研究是脊柱微创外科的一个热点。
