2型糖尿病合并微血管病变生长激素水平与胰岛素分泌功能分析

摘    要:目的探讨2型糖尿病微血管病变患者生长激素(GH)水平与胰岛分泌功能情况及相关因素.方法测定86例2型糖尿病患者,其中无合并微血管病变组45例和合并微血管病变组41例,以及正常对照组40例进行血糖(BG)、生长激素(GH)、胰岛素(INS)、HbA1c、TC、TG、LDL及HDL检测,计算胰岛素敏感指数(IAI),胰岛素分泌功能指数(IS).结果糖尿病两组各时相血糖值及HbA1c均显著高于正常对照组,合并微血管病变组,HbA1c、FGH、TC、TG及LDL显著高于无微血管病变组及正常对照组(P<0.01),胰岛素分泌功能指数显著低于无微血管病变组(P<0.01).Pea体on相关分析显示,FGH与HbA1c、TC及LDL呈显著正相关,而与IS呈显著负相关.HbA1c与TC、TG呈显著正相关,而与IS呈显著负相关.结论2型糖尿病微血管病变患者存在严重的持续性高血糖和脂质代谢紊乱.FGH水平升高,胰岛素分泌功能障碍对糖尿病微血管病变的发生、发展有重要的临床意义.

Abstract:[Objective] To investigate the relationship between serum growth hormone(GH)levels and the insulin secretory function in type 2 diabetes with microangiopathy.[Methods] 86 type 2 diabetes(including 45 without microangiopathy and 41 with microangiopathy)and 40 healthy objects were tested for BG,GH,INS,HbA1c and lipid.The insulin sensitivity index(IAI)and insulin secretion index(IS)had been calculated.[Results] The results showed that the values of blood sugar in each time-phase and HbA1c in both type 2 diabetes groups were significantly higher than those healthy objects.In groups patients of type 2 diabetes with microangiopathy,the serum levels HbA1c,FGH,TC,TG,LDL were significantly higher than those without microangipathy and healthy objects(P <0.01).IS in type 2 diabetes with microangiopathy was significantly lower than that without microangiopathy(P <0.01).The results of correlated analysis with Pearson showed that FGH was significantly positive correlated with HbA1c,TC,LDL.FGH was significantly negatively correlated with insulin secretion index.HbA1c was significantly positive correlated with TC,TG.HbA1c was significantly negatively correlated with IS.[Conclusion] These results suggested that type 2 diabetes with microangiopathy has persistent elevated levels of blood sugar and a disorder of lipid metabolism.The elevated levels of FGH and insulin secretion deficiency have the significance of clinic in the generation and development of diabetic microangiopathy.
