异体皮覆盖深Ⅱ度烧伤刮痂创面实验研究

摘    要:目的通过实验研究刮痂术及新鲜异体皮覆盖烧伤创面在深Ⅱ度烧伤早期创面组织进行性损伤中的积极作用。方法对48只SD大白鼠86处深Ⅱ度烧伤创面行刮痂术及新鲜异体皮覆盖,用组织病理学观察真皮层、炎性反应、上皮生长、愈后瘢痕情况。结果伤后8h内行刮痂术及新鲜异体皮覆盖创面,淤滞区有逆转,创面变浅,上皮生长快,愈后瘢痕轻;而未行刮痂术组,淤滞区无逆转,创面逐渐加深,炎性反应较重,上皮生长相对慢,愈后瘢痕较重。结论刮痂术及新鲜异体皮覆盖烧伤创面能使淤滞区逆转,创面变浅,渗出及炎性反应减少,对防止深Ⅱ度创面组织进行性损伤有积极意义。

Abstract:Objective To explore the positive effect of tangential excision and fresh allogenous skin transplanting on the progressive injury of deep partial thicknes burn wound in early stage in animal experiment.Methods Total 86 deep partial thickness burn wounds performing allogenous skin tansplanting in 48 SD rats were measured,observing dermis layer,inflammatory response,epithelium growing and cicatrization.Results Transplanting fresh allogenous skin within 8 hours after tangential excision could make stasis zone inversed,wound controlled and epithelium growing quickened,but scars was not obvious;while in control group,wound deepen gradually,inflammatory response and scars formation aggravated and epithelium grew slowly.Conclusions It is recommended that fresh allogenous skin is applied to the treatment of burn wound on the basis of tangential excision for it makes stasis zone inversed,wound controlled,exudation and inflammatory response decreased.It has a certain positive significance on preventing the progressive injury of deep partial thickness burn wound.
