切开复位内固定治疗髋臼骨折

目的:探讨切开复位内固定在治疗髋臼骨折中的临床应用价值。方法 :采用切开复位内固定治疗有移位的髋臼骨折共 15例。手术入路采用 Kocher- L angenbeck(K- L)入路 7例 ,髂腹股沟入路 3例 ,髂股加 K- L 入路 3例 ,三射线入路 2例。结果 :解剖复位 10例 ,满意 3例 ,差 2例。随访功能评分优 7例 ,良 3例 ,可 2例 ,差 3例。结论 :切开复位内固定已成为多数有移位髋臼骨折的治疗标准 ,L etournel分类对手术入路有很好的临床应用价值

Acetabular fractures treated with open reduction and internal fixation

Objective: To summarize clinic application and experience for the treatment of acetabular fracture with open reduction and internal fixation. Methods: From 1997 to now, 15 cases with displaced acetabular fractures were treated with open reduction and internal fixation. Fractures were fixed through the common classic approaches to the acetabulum: in 7 patients, the posterior Kocher Langenbeck approach was taken. The ilioinguinal approach was used in 3, the combined posterior (Kocher Langbenbeck) and anterior (ilioinguinal) approach in 3, and the triradiate approach was used in 2. Results: In 10 patients anatomic reduction was achieved; good reduction was obtained in 3, whereas in 2 patients, the reduction was incomplete. With an average of 19 months following up, 7 patients had an excellent result; 3 patients had a good result; 2 patients had a fair result; 3 patients had a poor result. Conclusions: Open reduction and internal fixation has become the standard of care for the treatment of most displaced acetabular fracture. Moreover, it is also important to actively preclude complications, which is helpful to promote the functional recover of patient.
