摘    要:目的:分析重症监护病房(ICU)呼吸机相关性肺炎(VAP)发生的相关危险因素,为探讨VAP的预防提供参考。方法:对80例行人工机械通气的ICU住院患者的临床资料进行回顾性分析,对VAP发生的相关危险因素进行Logistic回归分析,筛选VAP发生的相关危险因素。结果:单因素分析结果显示,机械通气时间、意识状态、误吸、H2-受体拮抗剂或酸剂、激素、抗菌素联合应用与VAP发生有关;多因素Logistic回归法分析表明,抗菌素联合应用≥2种、机械通气时间≥3d、误吸是VAP发生的危险因素。结论:抗菌素联合应用≥2种、机械通气时间≥3d、误吸是VAP发生的危险因素,对这些危险因素加以重点评估和合理控制可以预防VAP的发生。

Abstract:Objective To investigate the risk factors of incidence of ventilater associated pneumonia (VAP) in ICU and provide reference for prevention of VAP.Method Total 80 cases of ICU patients with artificial mechanical ventilation were studied,analyzing and screening the risk factors of VAP by Logistic.Results The single-factor analysis showed that the mechanical ventilation time,state of consciousness,aspiration,H2-receptor antagonist or acid agents,hormones and antibiotic combination had certain degree of contact with incidence of VAP.Multivariate Logistic regression analysis showed that the antibiotic combination (more than or equal to two species) ,the mechanical ventilation time (more than or equal to 3 days) and aspiration were risk factors of incidence of VAP.Conclusion The antibiotic combination (more than or equal to two species) ,the mechanical ventilation time (more than or equal to 3 days) and aspiration are risk factors of incidence of VAP,assessment and control these risk factors can prevent the incidence of VAP.
