膜式液基超薄细胞检测诊断宫颈病变的临床价值

目的:提高子宫颈病变检出率。方法:将324例慢性宫颈炎患者随机分成两组,TCT组186例和巴氏组138例,TCT组采用膜式液基超薄细胞检测(TCT)检测宫颈细胞,巴氏组采用宫颈涂片巴氏染色法检测宫颈细胞。比较TCT组与巴氏组的宫颈病变检出率和涂片满意率。结果:TCT组C INⅠ检出16例,占8.60%,巴氏组CINⅠ检出4例,占2.90%,两组检出率比较有显著差异(P<0.05)。TCT组CINⅡ-Ⅲ检出11例,占5.91%,巴氏组CINⅡ-Ⅲ检出2例,占1.45%,两组检出率比较有显著差异(P<0.05)。TCT组标本满意率为92.47%,巴氏组标本满意率为72.46%,两组比较有极显著差异(P<0.001)。结论:TCT可替代宫颈涂片巴氏染色法,减少漏诊,提高细胞学检查质量,能及时发现宫颈早期病变,是防止宫颈癌发生的有力措施。

The clinical value of ThinPrep Cytology Test (TCT) for diagnosis of cervical lesions

Objective:To improve the detectable rate of cervical lesions.Methods:324 chronic cervicitis patients were randomly divided into TCT group(n=186) and Papanicolaou group(n=138).TCT group were detected using ThinPrep Cytology Test,Papanicolaou group were detected using cervical cells Pap smear,and compared with the rate of finding with the cervical lesions and the rate of content smear of TCT group and Papanicolaou group.Results:16(8.60%) of 186 TCT group were cervical intraepithelial neoplasia-Ⅰ(CIN-Ⅰ),4(2.90%) of 138 Papanicolaou group were CIN-Ⅰ,the difference was statistically significant(P<0.05).11(5.91%) of 186 TCT group were CIN Ⅱ~Ⅲ,2(1.45%) of 138 Papanicolaou group were CIN-Ⅱ-Ⅲ,the difference was statistically significant(P<0.05).The rate of content smear of TCT group was 92.47%,the rate of content smear of Papanicolaou group was 72.46%,the difference was statistically significant(P<0.001).Conclusion:TCT could replace Papanicolaou Cytology Test,decrease the miss diagnosis rate,improve the quality of cytological examination,find out the early cervical lesions as soon as possible,and may be an important method of prophylaxis and treatment of early cervical carcinoma.
