血小板输注无效的三种处理方法

目的：探讨血小板输注无效的临床常用3种治疗手段,以确定较为有效的方案。方法：将确定为血小板输注无效的72例白血病患者,随机分为3组：加大剂量组（加大血小板输注量）、洗涤组（输注涤血小板）、免疫球蛋白组（大剂量静脉注射免疫球蛋白滴注后输注）,观察3组患者输注浓缩血小板后Plt改变,校正血小板增值（CCI）及出血症状的改善效果。结果：加大剂量组Plt有升高,但下降较快;洗涤组Plt无明显变化;免疫球蛋白组Plt明显升高,P〈0.05,与输注前比较差异有显著性,CCI值表示临床有效;免疫球蛋白组与加大剂量组能改善出血症状。结论：大剂量免疫球蛋白滴注后,输注机采浓缩血小板与加大输注机采浓缩血小板量均有改善血小板输注无效的作用,以前者作用明显,而输注经洗涤血小板则无明显疗效。

Three Treatments for Patients with Platelet Transfusion Refractoriness

Objective ： To discuss which one was the best of three treatments in treating platelet transfusion refractoriness. Method： 72 leukemia diseases patients were defined to platelet transfusion refractoriness, and were randomly decided into three groups： increase dosage group , washing group and immunoglobulin group. After infusing concentration platelet in the three groups patients, we observe the platelet in the three groups patients and the platelet change, adjusted CI and the result in hemorrhage. Result： Platelet had increased in the increase dosage group, but drop quickly. was increased in the immunoglobulin group （ P 〈 0.05 ） , that it had therapeutic effect. The symptom immunoglobulin group. Conclusion： Infusi Platelet had no change in washing group. Platelet which had significant difference, and CI indicated of hemorrhage was improved in the increase dosage group and the ng concentration platelet after giving large dosage immunoglobulin and increasing dosage of platelet could improve platelet transfusion refractoriness, the former was the better , but infuse washing platelet had no significant effects.
