实体性脑干肿瘤的显微手术治疗

本文报道显微手术治疗２３例实体性脑干肿瘤：肿瘤全切除１３例，次全切除８例，大部切除２例。出院时，神经系统功能好转者１８例（７８．３％）．不变２例．加重２例。死亡１例。随访结果也表明效果良好，好转率为７７．８％（１４／１８）。显示了显微手术的优越性；认为对脑干肿瘤应采取积极的态度，即原则上应争取手术治疗；描述了不同手术入路和显微手术方法：阐述了术后监护和并发症（特别是呼吸功能障碍）处理的重要性。

Microsurgery of solid brainstem tumor

Microsurgery of 23 cases with solid brainstem tumor is reported. Tumors were totally removed in 13 cases, subtotally removed in 8 and major part removed in 2. Upon discharge, symptoms and signs were improved in 18 cases (78.3%), stable in 2 and aggravated in 2. Postoperative death took place in one patient. There was improvement rate of 77.8%(14/18) from follow up. Superiority of microsurgery was demonstrated. We propose that more energetic attitude to aggressive manner, that is, should be placed to the surgical treatment for brainstem tumor. Different operative approaches and microsurgical techniques were described. Importances of postoperative intensive care and treatment for postoperative complications, particularly respiratory disturbance,were emphasized.
