一期后前路减压融合内固定治疗慢性颈脊髓钳夹型损伤

摘    要:目的评价一期后前路联合减压融合内固定治疗慢性颈脊髓钳夹型损伤的临床可行性及疗效。方法自2005年3月~2009年7月对23例慢性颈脊髓钳夹型损伤行一期后前路减压植骨融合内固定术治疗,按照JOA评分标准评定患者神经功能状态。结果 23例获得随访18~56个月,平均24.6个月,JOA评分提高到(16.1±1.2)分,症状改善率88.6%。结论一期后前路联合减压植骨内固定术治疗慢性颈脊髓钳夹型损伤减压彻底,神经功能恢复好,在条件允许的情况下,应作为该类患者优先选择的手术方式。

Anterior and posterior approach decompression with fusion and fixation in one stage operation for treatment of pinching injure of chronic cervical spondylotic myelopathy

Abstract:Objective To investigate the clinical feasibility and effect of anterior and posterior approach decompression with fusion and fixation in one stage operation for treatment of pinching injury of chronic cervical spondylotic myelopathy.Methods From Mar.2005 to Jul.2009,23 cases of pinching injury of chronic cervical spondylotic myelopathy were treated with anterior and posterior approach decompression with fusion and fixation in one stage operation.Neural functionality was evaluated according to JOA scoring system.Results Twenty three cases were followed up for 18 to 56 months,with an average of 24.6 months.JOA score raised to(16.1+1.2).The average rates of improvement were 88.6%.Conclusion Anterior and posterior approach decompression with fusion and fixation in one stage operation for treatment of pinching injury of chronic cervical spondylotic myelopathy can make complete decompression with good recovery of the nervous function.It should be the first choice for the patients with this disease if the condition permits.
