颈前路钩椎关节减压和改良植骨术(附61例报告)

目的评估钩椎关节减压改良植骨术在颈椎手术中的应用价值,探讨其手术适应证。方法回顾性分析61例接受颈前路钩椎关节减压改良植骨术治疗的颈椎病,利用NDI和VAS评分评价疗效,X线片及CT检查评估融合情况。结果所有患者平均随访16.5个月(12~21个月),术后症状均得到改善,术后3、6个月NDI及VAS评分均显著改善(P<0.001)。术后18个月58例(95.1%)融合。结论钩椎关节减压改良植骨术治疗颈椎病可以提高融合率,避免常规术式中出现的供骨区并发症问题。该术式尤其适用于伴有神经根管狭窄的神经根型颈椎病或者混合型颈椎病。

Unicinate decompression and modified bone graft technique for anterior cervical spine surgery:a study based on 61 cases

Objective To assess the technique of uncinate decompression and modified bone graft(UDMBG) for anterior cervical surgery and investigate the indications of this technique.Methods Sixty-one patients with cervical spondylosis who underwent UDMBG were reviewed.NDI and VAS scores were used to evaluate the efficacy and X-ray photography and CT for the fusion rate.Results All patients were followed up for 12 to 21 months,with 16.5 months on average.Symptoms were improved in all patients.Both NDI scores and the VAS scores were significantly improved 3 months and 6 months after operation(P <0.001).The spinal fusion was achieved in 58 patients(91.5%) 18 months after operation.Conclusion The uncinate decompression and modified bone graft can improve the fusion rate for cervical spondylosis.It can avoid the complications in the bone harvest area commonly occurred after the conventional procedures.This modified surgical procedure is applicable for the cervical spondylosis,especially combined with the osteophyte of the uncovertebral joint and neural foraminal stenosis.
