健脾生血颗粒防治婴幼儿铁缺乏及缺铁性贫血的疗效观察

目的：观察探讨健脾生血颗粒防治婴幼儿铁缺乏及缺铁性贫血的临床疗效及安全性。方法：选取2017年5月至2019年5月华中科技大学同济医学院附属武汉儿童医院产科出生的早产、低体质量、双胎或多胎、二胎新生儿96例作为研究对象，按随机分配原则分为观察组和对照组，每组48例。对照组给予蛋白琥珀酸铁口服液治疗，观察组给予健脾生血颗粒治疗。治疗12个月后对铁缺乏和缺铁性贫血患病率进行比较，对2组婴幼儿治疗前和治疗第3个月、第6个月、第12个月后的血红蛋白(Hb)检查进行比较，对治疗前和治疗12个月后血清铁蛋白(SF)水平检查比较，并对2组婴幼儿治疗12个月后主要症状体征发生率、治疗前及治疗12个月后神经行为发育评分及治疗期间不良反应发生率进行统计比较。结果：治疗12个月后观察组铁缺乏和缺铁性贫血患病率均低于对照组(P<0.05)。治疗第3个月、第6个月、第12个月2组婴幼儿Hb跟治疗前比较差异有统计学意义(P<0.05)。治疗12个月后2组婴幼儿SF水平与治疗前比较显著升高，且观察组高于对照组(P<0.05)。在治疗12个月后观察组主要症状体征总发生率低于对照组(P<0.05)。治疗12个月时2组婴幼儿神经行为测定评分(NBNA)、运动发育评分(PDI)、智力发育评分(MPI)与治疗前比较均有显著提高，且观察组各项评分均高于对照组(P<0.05)。结论：健脾生血颗粒防治婴幼儿铁缺乏及缺铁性贫血的疗效较好，能够升高机体Hb、SF水平，改善婴幼儿的主要不良症状体征和神经行为发育评分，且治疗安全性良好，值得临床推荐。

Observation on the Curative Effects of Jianpi Shengxue Granule in Preventing and Treatment of Iron Deficiency and Iron Deficiency Anemia in Infants

To observe the clinical efficacy and safety of Jianpi Shengxue Granule in preventing iron deficiency and iron deficiency anemia in infants.Methods:From May 2017 to May 2019,a total of 96 premature,low-weight,twin or multiple-born,second-born neonates born in Wuhan Children''s Hospital(Wuhan Maternal and Child Healthcare Hospital)were selected as the research subjects,and were divided into an observation group and a control group according to the principle of random distribution,with 48 cases in each group.The control group was treated with iron protein succinate oral solution,and the observation group was treated with Jianpi Shengxue Granule.After 12 months of treatment,the rate of iron deficiency and iron deficiency anemia was compared,and hemoglobin(Hb)examination was performed before treatment and after 3 months,6 months,and 12 months after treatment in 2 groups of infants.Serum ferritin(SF)levels before and 12 months after treatment were compared.The incidence of main symptoms and signs of the 2 groups of infants and young children after 12 months of treatment,neurobehavioral development scores before and 12 months after treatment,and the incidence of adverse reactions during treatment were statistically compared.Results:After 12 months of treatment,the rate of iron deficiency and iron deficiency anemia in the observation group were lower than those in the control group(P<0.05).Compared with those before treatment,the Hb of infants in the 3rd,6th and 12th months of treatment were significantly different(P<0.05),and there was significant difference between the two groups(P<0.05).Compared with those before treatment,the SF levels of the infants in the 2 groups were significantly increased after 12 months of treatment,and the observation group was higher than the control group(P<0.05).The total incidence of major symptoms and signs in the observation group was lower than that in the control group after 12 months of treatment(P<0.05).At 12 months of treatment,the 2 groups of infants'' neurobehavioral measurement score(NBNA),motor development score(PDI),and mental development score(MPI)were significantly improved compared with those before treatment,and the scores of the observation group were higher than those of the control group(P<0.05).Conclusion:Jianpi Shengxue Granule is effective in preventing iron deficiency and iron deficiency anemia in infants and young children.It can raise the body''s Hb and SF levels,improve the main adverse symptoms and signs and neurobehavioral development scores of infants and young children,and it is safe and is worthy of being recommended.
