98例创伤性肝破裂的诊治效果研究简

目的总结不同分级的创伤性肝破裂的诊断和治疗方法，为提高其临床救治率提供可靠依据。方法选择衢州市人民医院2009年5月-2013年3月救治的98例创伤性肝破裂患者作为研究对象，总结分析诊治经验、手术与非手术治疗方法救治结果。结果98例患者中有13例采用非手术治疗，其中2例未能成功救治转为手术治疗，均治愈出院；85例直接采用手术治疗，1例患者术后死于循环衰竭，6例出现手术并发症，但经积极治疗后均恢复正常。结论手术仍是治疗创伤性肝破裂的主要措施，术中可根据患者肝脏破裂程度采取不同的手术方案，非手术治疗仅适用于肝脏破裂程度较轻的患者。

Effect research of diagnosis and treatment of 98 cases with traumatic hepatic rupture

Objective To summarize the diagnosis and treatment of traumatic hepatic rupture with different grading, and provide a reliable basis for improving of clinical cure rate. Methods 98 cases with traumatic hepatic rupture from May 2009 to March 2013 in the People＇s Hospital of Quzhou City were selected as the study objects, the experience of diagnosis and ＇tr~atment~ surgical and non-surgical treatment, and treatment results were summary analyzed. Results 13 cases of 98 patients were treated with non-surgical treatment, including 2 cases failed to treat and converted to surgical treatment, all patients were cured after discharge; 85 cases were treated by surgery directly, 1 case died of circulatory failure, 6 cases with surgical complications, but recovered after active treatment. Conclusion Surgical is still the main treatment of traumatic hepatic rupture, can be took different operative plan according to the degree of ruptured liver, non-surgical treatment should be applied only in patients with light liver rupture.
