| 摘 要： | 目的探讨肾脏集合管癌（collecting duct carcinoma of kidney，CDC）的临床病理特点、鉴别诊断及预后。方法对8例肾脏集合管癌病理学、免疫组化进行观察，并复习临床资料及相关文献。结果肿瘤好发于中老年男性（平均年龄50．8岁），临床常见腰痛、血尿。光镜下见瘤细胞呈靴钉状、不规则腺管状、乳头状结构，肿瘤有明显的间质反应。免疫组化：瘤细胞通常表达高分子量角蛋白、vimentin。随访2例死亡。结论肾脏集合管癌有独特的临床及形态学特征，其预后跟肿瘤的形态密切相关，诊断主要靠形态学及免疫组化。

| Abstract: | Objective To observe the clinicopathological features of collecting duct carcinoma of kidney(CDC).Methods The clinical records and results of HE,immunohistochemical examination of 8 CDC cases were analyzed.Results The average age of 8 cases was 50.8 years and male dominated.The major symptoms were hematuria and flank pain.In histopathologic examinations tumor cells displayed hobnail-like shape and diffuse tubular or tubular-papillary pattern with desmoplastic stromal reaction.Immunohistochemical staining showed positive for CK7,EMA and vimentin in tumor cells.Conclusion CDC can be differentiated from other kidney tumors based on HE and immunohistochemical staining. |
