动脉栓塞术在危重症十二指肠球部溃疡大出血的治疗价值

目的探讨动脉栓塞术在危重症十二指肠球部溃疡大出血的治疗价值和治疗安全性。方法通过对本院2008年1月至2011年1月由于十二指肠溃疡导致大出血使用内科保守治疗无效患者11例,采用经右股动脉入路插管行超选择性胃十二指肠动脉或和胃网膜右动脉造影,判定出血具体动脉后使用合适大小明胶海绵颗粒和不锈钢圈尽量接近出血动脉栓塞,评估术后的临床疗效。结果 11例患者均一次栓塞成功率100%,术后无明显并发症发生。结论动脉栓塞术对危重症十二指肠球部溃疡大出血具有止血迅速、疗效显著、安全性高的特点,临床上建议进一步推广运用。

Therapeutic value of artery embolization in treatment of critically ill patients with duodenal ulcer bleeding

Objective To investigate therapeutic safety and therapeutic value of the arterial embolization in treatment of critically ill patients with duodenal ulcer bleeding.Methods The hospital in January 2008 to January 2011 led to massive hemorrhage because of duodenal ulcer with conservative medical treatment fails in patients 11 cases,the use of road through the right femoral artery catheter into the gastroduodenal artery line of super-selective or the right gastroepiploic artery and angiography to determine the specific arterial bleeding after use of a suitable size of gelatin sponge particles and stainless steel ring as close to bleeding artery embolization,assessed the clinical efficacy of surgery.Results Totally 11 patients one success rate of embolization in 100%,no significant postoperative complications.Conclusions Arterial embolization in treatment of critically ill patients with duodenal ulcer bleeding can stop bleeding rapidly,it has a significant effect,high security features,the clinical use recommended further promotion.
