64层螺旋CT冠状动脉成像在诊断冠状动脉疾病中的临床应用

摘    要:目的评价64层螺旋CT冠状动脉成像(64SCTA)在诊断冠状动脉疾病中的应用价值。方法收集60例行64层螺旋CT冠状动脉成像并在短期内做冠状动脉造影(CAG)患者,以CAG的诊断结果作为金标准,将2种方法评价冠状动脉的结果进行对比,探讨64SCT冠状动脉成像检出冠状动脉狭窄病变的准确性。结果 60例患者显示的842个冠状动脉节段中,可用于评估的为751个节段,可评估率89.2%。64SCTA诊断冠状动脉狭窄程度<50%的敏感性为77.4%,≥50%的敏感性为89.7%,特异性为92.8%,阳性预测值为85.4%,阴性预测值为95.8%,准确率90.9%。结论 64SCTA对于冠状动脉狭窄的评估具有较高敏感性和特异性,具有较好的临床应用价值。

Abstract:Objective To evaluate the application values of 64-slice spiral computed tomography angiography(64SCTA) in the diagnosis of coronary artery disease(CAD).Methods Totally 60 patients with suspected coronary artery stenosis were analyzed by 64SCTA and coronary angiography(CAG).The CAG was regarded as the golden standard to evaluate the results and accuracy of 64SCTA in the detection of coronary artery stenosis.Results Totally 751 segments of the 842 coronary artery segments in 60 patients could be visualized by 64-slice spiral CT,the appreciable rate was 89.2%.According to gold standard of selective coronary arteriongraphy,the sensibility,specificity,positive predictive value,negative predictive value and accuracy rate to identify coronary artery stenosis by 64SCTA were 77.4%(for coronary artery stenosis<50%),89.7%(for coronary artery stenosis≥50%),92.8%,85.4%,95.8% and 90.9% respectively.Conclusions As a non-invasive method,64SCTA coronary artery angiography has high sensitivity as well as good specificity for the evaluation of the stenosis of coronary artery and has good clinical value.
