足月胎膜早破至分娩时间对妊娠结局的影响

目的观察孕足月胎膜早破时间至分娩的时间间隔对妊娠结局的影响。方法统计深圳市第六人民医院2010年10月至2011年10月间入院分娩之足月胎膜早破患者638例。按胎膜早破时间至分娩的时间间隔分组。统计各组产妇的剖宫产率、产后出血量、绒毛膜羊膜炎发生率、胎儿窘迫发生率、新生儿出生后Apgar评分以及新生儿收住院率。结果各组间产后出血、胎儿窘迫发生率及新生儿Apgar评分差异无统计学意义(χ2=8.71,P=0.084)。胎膜破裂至分娩时间≥48h组的剖宫产率为46.8%,显著高于破水至分娩时间<24h组,差异有统计学意义(χ2=4.66,P=0.047)。胎膜破裂至分娩时间≥48h组的绒毛膜羊膜炎发生率、剖宫产率及新生儿收住院率均显著高于破水时间<24h组(χ2=3.94,P=0.011)。结论孕足月胎膜早破至分娩时间超过48h绒毛膜羊膜炎、剖宫产率及新生儿并发症的发生率增加。

Term premature rupture of membranes to delivery time on pregnancy outcome

Objective To observe the pregnancy term premature rupture of membranes time to delivery time interval on pregnancy outcome.Methods Statistics Shenzhen City People’s Hospital from October 2010 to October 2011 deliveries admitted patients with term premature rupture of membranes 638 cases.By premature rupture of membranes time to delivery time interval grouping.The statistics of each group of maternal rate of cesarean section,postpartum hemorrhage,chorioamnionitis,the incidence of fetal distress rate to close after birth Apgar scores and neonatal hospitalization rates.Results Among the groups postpartum bleeding,fetal distress and neonatal Apgar score differences not statistically significant(χ2=8.71,P=0.084).Rupture of membranes to delivery time ≥ 48h group cesarean section rate was 46.8%,significantly higher than the rupture of membranes to delivery time <24h group,the difference was statistically significant(χ2=4.66,P=0.047).Rupture of membranes,chorioamnionitis to delivery time ≥ 48 h group,the incidence of cesarean section rate and neonatal received hospitalization were significantly higher than the water break time <24 h group(χ2=3.94,P=0.011).Conclusions The incidence of pregnancy term premature rupture of membranes to delivery time increased more than 48h chorioamnionitis,cesarean section rate and neonatal complications.Term premature rupture of membranes in patients admitted to hospital should be actively induced labor.Shorten the fetal membranes rupture to the time of completion of delivery outcome is essential to improve the pregnancy.
