肝细胞内HBV抗原表达与肝损伤关系的研究

采用PAP免疫组化方法,对96例肝活检标本进行肝细胞内HBsAg和HBcAg免疫反应性定位,探讨肝细胞损伤程度与肝细胞内HBsAg和HBcAg表达的关系。结果显示:慢性活动型肝炎和肝硬变患者,肝细胞内HBsAg表达以膜型为主,HBcAg以浆膜型为主。慢性迁延型肝炎和HBV携带者,肝细胞内HBsAg以包涵体型为主,HBcAg则以核型为主。提示:肝细胞膜上表达的HBsAg和HBcAg可作为靶抗原,导致肝损伤。

Study of the relation between the intrahepatocyte expression of HBV antigen and the liver injury

The experiments in this study were done with the PAP immunohistochemistry method to study theintrahepatic immunoreactivity of HBsAg and HBcAg in 96 biopsy samples of liver to find out the rela-tion between the intrahepatocyte expression of HBsAg and HBcAg and the liver injury. The resultsshowed that in the patients with chronic active hepatitis (CAH)and liver cirrhosis,in which the liverswere seriously injured,the major expression of HBsAg was of membrane pattern,while that of HBcAgwas of membrane and Plasma ones. In the patients with chronic persistent hepatitis(CPH), and theHBV carriers,HBsAg in hepatocytes mainly appeared in the form of inclusion bodies,and only a fewmembrane patterns could be seen. Most of HBcAgs appeared in nucleus pattern. Plasma and membranepatterns were very few. The difference of the membrane pattern expression of HBV antigens betweenthe group of CAH and cirrhosis, and the group of CPH and the carriers were significant (P<0. 05).The results suggested that HBsAgs and HBcAgs expressing on the cell membrane might act as targetantigens to lead to liver injury.
