经内镜置入支架治疗高龄难治性胆总管结石40例

目的 探讨经内镜置入支架治疗高龄难治性胆总管结石的疗效.方法 对40例高龄、伴基础疾病、内镜下取石困难的患者行胆道塑料支架置入(endoscopic retrograde bilioduodenal drainage,ERBD),并随访支架的通畅情况及结石变化情况.结果 术后半年支架通畅率达90.6％ (29/32),1年通畅率达77.8％(21/27),2年通畅率达64.3％(9/14)；术后复查患者35例结石均有不同程度变小或变为多块,经再次取石成功.结论 对高龄难治性胆管结石经ERBD治疗疗效确切.

Endoscopic stent implantation in 40 elderly patients with intractable choledocholithiasis

Objective To explore the therapeutic effect of endoscopic stent implantation for elderly patients with intractable choledocholithiasis. Methods Forty elderly patients with basic diseases whose stones were difficult to remove underwent endoscopic retrograde bilioduodenal drainage(ERBD) and were followed up for stent patency and changes of the stones. Results Stents patency rate of half a year,1 year,2 years after operation were 90.6%(29/32),77.8%(21/27) and 64.3%(9/14) respectively.35 patients who came for subsequent visit had smaller or dispersed stones which were successfully removed by ERCP afterwards. Conclusion ERBD has definite therapeutic effect on intractable choledocholithiasis.
