RMI和Ferrazzi鉴别附件肿瘤良恶性的价值和内镜处理

目的:评价恶性风险指数(RMI)和Ferrazzi评分系统鉴别附件肿瘤良恶性的价值,利于内镜处理。方法:2008年10月～2010年6月期间收治的附件肿块患者331例,以RMI=150、Ferrazzi=8作为良恶性鉴别临界点,决定是否进行内镜处理,计算两者预测附件恶性肿瘤的敏感度、特异度、阳性预测值(PPV)、阴性预测值(NPV),对诊断符合率进行比较。结果:对附件肿物良恶性鉴别中,RMI和Ferrazzi有较高准确率。以150作为RMI临界值,其敏感度80.00%,特异度96.35%,PPV 68.57%,NPV 97.97%;以8作为Ferrazzi临界值,其敏感度86.67%,特异度95.54%,PPV 74.29%,NPV 97.97%。结论:RMI和Ferrazzi对附件恶性肿瘤的诊断明显相关,且Ferrazzi优于RMI能更准确地预测恶性,且对临床附件肿块是否内镜处理具有重要的指导意义。

Values of RMI and Ferrazzi scoring system for differentiation of adnexal tumor character and endoscopic treatment

Objective:To evaluate the values of risk of malignancy index(RMI) and Ferrazzi scoring system for differentiation of tumor character to help endoscopic treatment.Methods:From October 2008 to June 2010,331 patients with adnexal masses were included in the study.Taking RMI=150 and ferrazzi =8 as critical points to identify adnexal tumor character and decide to perform endoscopic treatment or not;the sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic coincidence rate were calculated and compared between the two methods.Results:The accurate rates of RMI and Ferrazzi scoring system for identifying adnexal tumor character were high.Taking RMI=150 as a critical point,the sensitivity,specificity,positive predictive value,and negative predictive value were 80.00%,96.35%,68.57%,and 97.97%,respectively;taking ferrazzi=8 as a critical point,the sensitivity,specificity,positive predictive value,and negative predictive value were 86.67%,95.54%,74.29%,and 97.97%,respectively.Conclusion:RMI and ferrazzi were significantly associated with diagnosis of malignant adnexal tumors,and ferrazzi was more accurate than RMI scoring system for identifying adnexal tumor character,and it has guiding significance for taking endoscopic treatment or not for adnexal masses in clinic.
