秦皇岛地区妊娠期糖尿病发病现状及其对母婴的影响

目的:分析秦皇岛地区妊娠期糖代谢异常现状及其对母婴预后的影响。方法:对2009年1月～2011年5月在秦皇岛市妇幼保健院产科门诊就诊的16 787例孕妇进行50 g葡萄糖筛查试验(GCT),阳性者进一步行75 g葡萄糖耐量试验(OGTT),并观察其妊娠结局。结果:GCT阳性者2 807例,妊娠期糖尿病(GDM)发生率为2.21%,妊娠期糖耐量异常(GIGT)发生率为2.54%,GDM组剖宫产率、早产率、羊水过多及新生儿并发症明显高于50 g GCT正常组(P<0.05)。结论:GDM发病率较高,并可导致母婴并发症增加,因此对存在GDM高危因素的孕妇尤其应重视对GDM的早期诊断、早期治疗,以降低母婴并发症,改善妊娠结局。

Current situation of gestational diabetes mellitus in Qinhuangdao area and its impact on mothers and infants

Objective:To analyze the current situation of gestational impaired glucose metabolism in Qinhuangdao area and its impact on maternal and infantile prognosis. Methods:A total of 16 787 pregnant women from outpatient department of the hospital received 50 g glucose challenge test(GCT),then the positive pregnant women further received 75 g oral glucose tolerance test(OGTT),the pregnancy outcome was observed. Results:A total of 2 807 pregnant women were found with positive result of 50 g GCT,the morbidity of gestational diabetes mellitus(GDM) was 2.21%,and the morbidity of impaired glucose tolerance was 2.54%,the rate of cesarean section and morbidities of premature labor,polyhydramnions,and neonatal complications in GDM group were statistically significantly higher than those in non-GDM group(P<0.05). Conclusion:The morbidity of GDM was relatively high,which can increase maternal and infantile complications,so more attention should be paid to early diagnosis and early treatment of GDM for the pregnant women with high risk factors of GDM to reduce maternal and infantile complications and improve pregnancy outcome.
