两指标联合应用对早产分娩时间的预测价值

目的:探讨胰岛素样生长因子结合蛋白-1(insulin-like growth factor binding protein-1,IGFBP-1)、宫颈指数对早产距离分娩时间的预测价值。方法:选择91例先兆早产孕妇,检测宫颈分泌物中IGFBP-1,同时经会阴超声检测宫颈指数,随访至分娩,分析两指标对距离分娩时间的预测价值。结果:IGFBP-1、宫颈指数阳性组48 h内分娩的阳性预测值分别为21.7%(10/46)、21.9%(9/41),1周内分娩的阳性预测值分别为32.6%(15/46)、34.1%(14/41)。两指标联合应用可明显提高对早产距离分娩时间预测的准确性(P<0.05)。结论:宫颈分泌物IGFBP-1、宫颈指数单独应用对早产距离分娩时间预测的临床意义不大,而两指标联合应用可明显提高其预测准确性,可用于指导临床。

Value of joint application of two markers for prediction of delivery time for premature delivery

Objective:To explore the values of insulin-like growth factor binding protein-1(IGFBP-1) and cervical index for prediction of the interval time from diagnostic time of premature delivery and delivery.Methods:A total of 91 pregnant women with threatened premature labor were selected,the level of IGFBP-1 in cervical secretions was detected,and cervical index was measured by transvaginal ultrasonography,all the pregnant women were followed up till delivery,the values of two markers for prediction of the interval time from diagnostic time of premature delivery and delivery were analyzed.Results:In positive IGFBP-1 and cervical index group,the positive predictive values of delivery within 48 hours were 21.7%(10/46) and 21.9%(9/41),respectively;the positive predictive values of delivery within one weeks were 32.6%(15/46) and 34.1%(14/41),respectively.Joint application of the two markers can obviously improve the accuracy of predicting the interval time from diagnostic time of premature delivery and delivery(P<0.05).Conclusion:The clinical significances of single IGFBP-1 in cervical secretions and single cervical index for predicting the interval time from diagnostic time of premature delivery and delivery are small,joint application of two markers can obviously improve the accuracy of prediction,which can be used to guide clinical treatment.
