三维超声在慢性乙肝肝内增生结节及小肝癌诊断中的应用

目的对慢性乙肝肝内病变应用超声进行动态观察，探讨三维超声在慢性乙肝肝内病变发展过程中的价值。方法对实验室检查报告表面抗原（HBsAg）、核心抗体（HBcAb）、e抗原（HBeAg）均阳性的慢性乙肝患者进行肝脏二维超声检查、三维重建，分析图像特点。穿刺活检后，对肝内结节二维、三维超声检出数比较。结果122例HBsAg（＋），HBcAb（＋），HBeAg（＋）患者中，三维超声能较清晰显示肝内增生结节、小肝癌形状及与周边肝组织关系，三维超声对小肝癌的检出数较二维超声明显增多。结论超声能早期发现慢性乙肝肝内结节性病变，三维超声多切面、多角度检查提高了小肝癌诊断准确性。

Three-dimensional ultrasound in the diagnosis of chronic hepatic lesion caused by HBV

Objective To observe dynamically the chronic hepatic lesion caused by HBV with three-dimensional ultrasound and to detect the value of ultrasound especially three-dimensional ultrasound in the diagnosis of this chronic lesion.Methods Chronic hepatitis cases with HBsAg(+),HBcAb(+) and HBeAg(+) were examined by ultrasound and three-dimensional reconstruction.WTHZ]Results Within 122 cases of HBsAg(+),HBcAb(+) and HBeAg(+),the intra-hepatic hyperplastic nodule,shape of small hepatoma and relationship between tumor and ambient tissue were displayed clearly with three-dimensional ultrasound and the number of detection with three-dimensional ultrasound were much higher than those of two-dimensional ultrasound significantly. Concl- usion Ultrasound could find out small intra-hepatic node earlier,and three-dimensional ultrasound can enhance the accuracy in diagnosing small hepatocellular carcinoma.
