老年胆道疾病59例外科治疗体会

目的:探讨老年胆道病人的病史特点、手术适应症及手术时机,提高手术治疗效果.方法:回顾性分析经手术治疗的59例老年胆道病人的临床资料.结果:59例病人均经手术治疗,其中胆囊切除40例,胆囊切除加胆总管探查引流14例.单纯胆总管探查引流术3例.胆囊结石合并肠道肿瘤同时手术2例..并存病发生率84.74%(50/59),并发症发生率10.2%(6/59),死亡1例.结论:掌握手术适应症和手术时机,积极处理并存病,是减少老年胆道病人术后并发症,降低病死率的关键.

Surgical treatment of 59 aged patients with gallbladder disease

Objective To explore characteristics of medical history, operation indication and timing in aged patients with gallbladder disease and to improve operative efficacy. Methods Retrospective study was performed on clinical data of 59 aged patients with gallbladder disease from Jan. 2001 to Dec. 2005.Results 59 aged patients were operated, including 40 patients with pure cholecystectomy, 17 patients with choledochus drainge, 2 patients with cholelith and intestinal tumor. Concomitant incidence accounted for 84.7%(50/59). Complication prevalence was 10.2%(6/59). Mortality was 1.69%(1/59).Conclusion To master operative indication and timing and actively mange concomitant disease plays an important role in reducing post-operation complication decrease mortality of aged patients with gallbladder disease.
