摘    要:目的 探讨乳突根治术后真菌感染致反复不干耳的原因及治疗.方法对38例乳突根治术后因真菌感染不干耳患者的临床资料进行分析总结.结果 术后合并真菌感染原因中:27例因为面神经嵴过高、窦脑膜角深凹、遗留鼓膜穿孔等原因使术腔引流不畅所致;8例因为反复应用抗生素所致;3例因为糖尿病等全身性疾病免疫功能低下所致.27例行手术改善术腔通风引流,25例痊愈,2例显效;其余11例经耳清洁及滴氟康唑治愈.结论 乳突根治术后术腔引流不畅、滥用抗生素是真菌感染致不干耳的重要因素,改善引流及局部滴抗真菌药可治愈.

Abstract:Objective To investigate the reasons and treatment of wet ear after radical mastoidectomy caused by fungal infection.Methods The clincal data of 38 cases of wet ear after radical mastoidectomy caused by fungal infection were analyzed and summarized.Results The reasons of fungal infections including: facial ridge too high,the sinodural angle deep and the tympanic membrane perforation that caused the inadequate drainage of operative cavus and wet ear in 27 cases;long-term administration of antibiotics in 8 cases;autoimmune diseases,such as diabetes,patient’s own immune system was relatively low in 3 cases.Twenty-seven cases were operated to improve the inadequate drainage of operative cavus,25 cases were recovered,2 cases indicated excellent efficacy;Eleven cases were cured by cleaning the ears and using fluconazol.Conclusion The main reasons of suppuration from ear canal after radical mastoidectomy caused by fungal infection are inadequate drainage of operative cavus and antibiotics abusing.It can be cured by improving the drainage of operative cavus and using antimycotic drugs.
