2型糖尿病患者多项生化指标在肾功能损伤中的诊断价值

目的：探讨同型半胱氨酸（Hcy）、血清胱抑素C（CysC）、糖化血红蛋白（HbA1c）和尿微量白蛋白（U-mAlb）联合检测对糖尿病早期肾脏损伤的诊断价值。方法分别检测122例2型糖尿病（T2DM）患者尿蛋白阴性（A组）62例，尿蛋白阳性（B组）60例]和健康体检者（NC组）65例的血清Hcy、CysC、HbA1c和U-mAlb的含量。结果 T2DM患者Hcy、CysC、HbA1c、U-mAlb的水平明显高于NC组（P〈0.01），且B组较A组升高明显（P〈0.01）。结论血清Hcy、CysC、HbA1c与U-mAlb的水平与T2DM患者的早期肾损伤程度密切相关，联合检测对于糖尿病肾病的早期诊断、早期治疗及监测有重要意义。

Diagnosis value of multiple biochemical criterions for renal function impairment in patients with type 2 diabetes mellitus

Objective To explore the diagnostic value of joint detection of homocystein （Hcy）, serum cystatin C （CysC）, glycosylated hemoglobin （HbA1c） and urine microalbumin （U-mAlb） in diagnosis of early diabetic renal impairment. Methods The levels of serum Hcy, CysC, HbA1c and U-mAlb were detected in 65 health people （NC group） and 122 patients with type 2 diabets mellitus （T2DM） 62 patients with negative urinary protein （group A）, 60 patients with positive urinary protein （group B）]. Results The levels of Hcy, CysC,HbA1c and U-mAlb in T2DM patients were obviously higher than those of NC group （P〈0.01）, and those levels increased more significantly in group B than in group A （P〈0.01）. Conclusion The levels of serum Hcy, CysC, HbA1c and U-mAlb are closely related to the early renal impairment in T2DM. The joint detection is important for the early diagnosis, control and monitoring of diabetic nephropathy.
