人乳头瘤病毒检测在宫颈上皮内瘤变诊断及随访中的意义

摘    要:目的探讨人乳头瘤病毒(HPV)在宫颈上皮内瘤变(CIN)中的表达,以及它们之间的相互关系,通过HPV检测尽早筛查出患宫颈上皮内瘤变的高危人群。并对术前术后随访中高危型HPV变化与病变残留复发情况进行动态观察及分析,以便采取有效的预防和治疗措施。方法对169例宫颈病变患者采用杂交捕获技术对HPV进行筛查,并通过阴道镜对可疑病灶多点活检。结果 169例宫颈病变患者中HPV阳性者68例(40.2%),阴性者101例(59.8%);病理活检慢性炎症73例(43.2%),宫颈癌及癌前病变96例(56.8%);HPV阳性患者的宫颈癌及癌前病变率为70.8%(68/96)。结论检测高危型HPV不仅可以发现现行的宫颈疾病患者,还可以发现存在高危风险的人群,是监测CIN的一个重要的指标;通过检测高危型HPV,有针对性地进行活组织病理检查,可有效地指导临床,有助于探讨CIN的发病机制。

Abstract:Objective To explore the expression of human papilloma virus (HPV) in cervical intraepithelial neoplasias (CIN) and their relation. To make screening of high risk group with CIN through HPV detection. To dynamically observe and analyze the change of high risk HPV and recurrent residual lesion, in order to take effective prevention and treatment measures.Methods A total of 169 CIN patients received HPV screening by hybrid capture technique, and biopsy for suspicious lesions by colposcope.Results Among the 169 CIN cases, there were 68 HPV positive cases (40.2%), 101 negative cases (59.8%), and 73 cases with chronic cervicitis (43.2%), 96 cases with cervical cancer and precancerous lesions (56.8%). The rate of cervical cancer and precancerous lesions in HPV positive cases was 70.8% (68/96).Conclusion The detection of high risk HPV can not only find CIN patients, but also discover high risk group. It is an important indicator for detecting CIN. This high risk HPV detection, along with targeted biopsy, can provide effective guidance, and is helpful for explore pathogenesis of CIN.
