川崎病凝血功能检测动态观察的临床意义探讨

摘    要:目的探讨川崎病患儿不同时期凝血功能与疾病进展的关系,为指导临床诊治、减少后天性心血管疾病提供科学依据。方法监测KD组和对照组患儿不同时期凝血功能及与冠状动脉病变发生的关系。结果①KD组患儿D-二聚体升高者极显著高于对照组(P〈0.01);②动态观察KD组与对照组D-二聚体升高比较〈7d组、7—14d组差异均极显著(P〈0.01),〉14d组无差异(P〉0.05);③CAD组与NCAD组D-二聚体升高比较无差异(P〉0.05);④动态观察CAD组与NCAD组D-二聚体升高比较〈7d组无差异(P〉0.05),7~14d组差异极显著(P〈0.01),〉14d组有显著差异(P〈0.05)。结论D-二聚体可作为KD早期诊断的重要指标之一,动态观察D-二聚体具有预测冠状动脉损伤的价值。

Abstract:Objective To research the relation between the progress of disease with coagulation function in different phases of kawasaki disease(KD) to provide foundation for guide the diagnosis and treatment and reduce the incidence of acquired cardiovascular disease. Methods Monitor the relation of progress of disease with coronary artery disease (CAD) in different phases of kawasaki disease (KD) and the control group o Resuits (1)D-dimer of the patients of KD were higher significantly than that of the control group ( P 〈 0. 01 ) . (2) According to the dynamic observation, the rise of D-dimer of patients of KD in less than 7 days or between 7 to 14 days had significant difference with the control group ( P 〈 0. 01 ) , and had not significant difference after 14 days ( P 〉 0.05 ). (3)The rise of D-dimer of patients of KD had not significant difference between CAD group and NCAD group( P 〉 0.05 ). (4)According to the dynamic observation, It had not significant difference between CAD group and NCAD group in less than 7 days ( P 〉 0. 05 ) , and had significant difference between 7 to 14 days ( P 〈 0. 01 ) or after 14 days ( P 〈 0. 05 ). Conclusion D-dimer can be one of important index for early diaguosis of KD, and the dynamic observation of D-dimer can predict the coronary artery lesions.
