戒烟对冠心病患者血脂、血液流变学及颈动脉内膜中层厚度的影响

目的 了解冠心病患者戒烟后不同时期的血脂度、血液流变学及颈动脉内膜中层厚度的变化。方法 90例经冠脉造影确诊为冠状动脉粥样硬化性心脏病的并合吸烟史男性患者，分为吸烟组，戒烟组，每组45例。分别在戒烟前、3个月及1年后对血脂、血液流变学指标及颈动脉内膜中层厚度进行组间及组内自身对照比较。结果 戒烟3个月后血脂开始变化，1年后较戒烟前及吸烟对照组有明显改善，差异有显著性(P<0 05)。1年后血液流变学大部分指标较戒烟前及吸烟对照组显著改善(P<0 05)。颈动脉内膜中层厚度戒烟3个月后较吸烟组未见有明显变化（0. 896 ±0. 213 vs 0. 918 ±0. 157 P>0.05），但12个月后指标较戒烟前及吸烟对照组显著改善（0. 806 ±0. 135 vs 0. 854 ±0. 162 P<0 05)。结论 戒烟对冠心病患者血脂，尤其对高密度脂蛋白胆固醇及血液流变学改善呈持续有益作用，较短时间戒烟不能改善颈动脉内膜厚度，但戒烟一年后颈动脉内膜厚度有逆转性改变。
