慢性阻塞性肺疾病痰中中性粒细胞表面CD11b表达及意义

摘    要:目的 :探讨慢性阻塞性肺疾病 (COPD)痰中中性粒细胞CD11b表达与气流阻塞间关系。方法 :对 16例COPD患者 ,15例慢性支气管炎患者和 15例健康志愿者诱导痰中细胞成份作染色分类并进行CD11b免疫组化检查 ,上清成份用酶联免疫法作白细胞介素 8(IL_8)水平测定。结果 :痰细胞分类中 ,COPD组中性粒细胞占 6 9.6 % ,与慢支组 (5 2 .9% ,P <0 .0 1)、健康组 (2 2 .2 % ,P <0 .0 1)比较差异有显著性。COPD组CD11b阳性率为 73 .5 % ,与慢支组 (5 3 .0 % ,P <0 .0 1)和健康组 (17.9% ,P <0 .0 1)比较差异有显著性。COPD组IL_8水平为 0 .72 0ng/ml,与慢支组 (0 .5 5 9ng/ml,P <0 .0 1)、健康组 (0 .16 1ng/ml,P <0 .0 1)比较差异有显著性。COPD组中CD11b、中性粒细胞、IL_8与一秒钟用力呼气容积 (FEV1)占预计值百分比成反比 (r =- 0 .77,P <0 .0 1;r =- 0 .79,P <0 .0 1;r=- 0 .85 ,P <0 .0 1)。且CD 11b与中性粒细胞、IL_8均成正相关 (r =0 .88,P <0 .0 1;r =0 .6 6 ,P <0 .0 1)。结论 :慢性阻塞性肺疾病中性粒细胞CD 11b表达增加与吸烟者发展为气流阻塞有关

Abstract:Objective:To determine the relationship between the expression of CD11b on sputum neutrophils and the airwayobstruction in patients with COPD. Methods: Cells in induced sputum from 16 COPD patients,15 chronic bronchitis patients and 15 healthy volunteers were stained and analysed micrscopically.The expression of CD11b was evaluated using immunohistochemistry techiniques. Interleukin_8(IL_8) in fluid_phase was measured with enzyme linked immunosorbent assay.Results: The percentage of neutrophils in sputum from COPD (69.6%) were significantly higher than those from chronic bronchitis (52.9%,P<0.01) and normal subjects (22.2%,P<0.001).CD11b_positive neutrophils in COPD (73.5%) significantly increased in comparison with chronic bronchitis (53.0%,P<0.01),normal subjects (17.9%,P<0.01).The level of IL_8 in COPD (0.720 ng/ml) was significantly higher than those in chronic bronchitis (0.559 ng/ml,P<0.01)normal subjets (0.161 ng/ml,P<0.01).FEV 1% correlated negatively with CD11b (r=-0.77,P<0.01),IL_8(r=-0.85,P<0.01) and the percentage of neutrophils(r=-0.79,P<0.01)in COPD.There was a signficant positive correlation between CD11b and IL_8(r=0.66,P<0.01) and the percentage of neutrophils(r=0.88,P<0.01).Conclusion: CD11b expresed on sputum neutrophils has a close relationship with the smokers who develop chronic obstrutive pulmonary disease.
