三种镇痛方法在人工流产术中的临床对比研究

目的:探讨人工流产中最佳镇痛方法。方法:将320例自愿行人工流产的早孕妇女随机分为A组(口服米索前列醇)、B组(丙泊酚注射)、C组(米索前列醇联合丙泊酚)、D组(常规人工流产),每组80例。对四组镇痛效果、宫颈松弛情况、术中出血量、子宫收缩幅度、手术时间、人工流产综合症及呼吸、循环系统情况进行比较。结果:A、B、C组镇痛、扩宮效果、手术时间与D组比较均有显著性差异(P(0.01),尤以C组效果最佳。子宫出血量及子宫收缩幅度四组无显著性差异(P(0.05)。应用米索前列醇后麻醉药物用量减少(P(0.05),术中副反应少(P(0.05)。结论:米索前列醇联合丙泊酚用于无痛人工流产,镇痛效果及宫颈松弛良好,副反应少且操作简单安全,值得临床推广使用。

A Comparsion Study on Three Different Analgesic Methods in Induced Abortion

Objective:To study the best antalgic methods in induced abortion.Methods:320 women with early pregnancy who asked for induced abortion were divided into four groups randomly.Women in group A were given 0.4mg misoprostol,women in group B were injected propofol into vein,women in group C were given misopostol and propofol,but women in group D were given nothing,every group was 80 cases.The degree of pain,the degree of cervical flab,amount of bleeding during the operation,the retractile extent of uterus,the operative time and side-effect were observed in four groups respectively.Results:The pain relief effect,the degree of cervical flab,the operative time,group A,B,C showed significant difference compared with group D,respectively(P<0.01),and the best one was the group C.but amount of bleeding and the retractile extent of uterus showed no significant difference among the four groups(P>0.05).the quantity of dope and the incidence,group C showed significant difference compared group B(P<0.05).Conclusion:Misoprostol combined with propofol is an effective way in inducing abortion.Its pain relief effectiveness and the degree of cervical flab are more positive than the others.It has less incidence and the operation is simple and safe,So,It is suitable for wide-spread use.
