纤维母-肌纤维母细胞肿瘤6例临床病理分析

目的：探索及鉴别近年内发现的三种起源于纤维母～肌纤维母细胞的软组织肿瘤，分别为侵袭性血管粘液瘤(Aggressive Angiomyxoms.AA)、血管肌纤维母细胞瘤(Angiomyofibroblastoma,AMF)及细胞性血管纤维瘤(Cellular Angiofibroma，CAF)的临床及病理特征。方法：采用光镜和免疫组织化学Envision法染色观察。结果：三种肿瘤均主要发生于外阴或阴道．多为生育期女性(本文报道1例为男性)，临床常误诊为外阴巴氏腺囊肿。光镜下均以间质细胞和丰富的血管两种成份为主，起源均为间质的纤维母细胞或肌纤维母细胞，免疫表型有部分相同性，但生物学行为及预后却不一致．故鉴别诊断十分必要。结论：三种肿瘤在预后方面存在差异．必须以临床、病理、免疫组化三结合的方法方能做出准确的诊断。

Fibroblastoma-myofibroblastoma: A Clinicopathologic Study of 6 Cases

Objective:To explore and distinguish the clinical and pathological character-isitics of three types of soft tissue tumorsoriginated from fibroblast-myofibroblaste,which were documented in recent years?These are include aggressive angiomyxoma (AA),angiomyofibroblas-toma (AMF )and cellular angiofibroma (CAF).Methods:Microscopic observation and immunohisto-chemistry staining were used.Results:These three types tumors all occur within the vulva or vagina.(except one case occurred in inguen)They usual occurred in reproductive aged women.(except one case occurred in man in our file).The clinical impression is often misdiagnosised as Bartholin cyst etc.Under microscopical observation the tumors are composed of prominent stromal and vascular component,which origin from fibroblast or myofibroblast and immounohistochemical phenotype are partly similarity.But their biological behaviour and prognosis are extremelly differ-ent.So their differential diagnosis are extremely important.Conclusions:We suggest that we must pay more attention to the pathological diagnosis,differential diagnosis and prognosis of above mentioned three types tumors which origin from fibroblast-myofibroblast.
