ICU机械通气患者不同吸痰方式的对比研究

目的对比观察开放式吸痰法和密闭式吸痰法对ICU机械通气患者的影响。方法选择ICU机械通气患者60例，随机分为A组和B组，每组30例。A组采用开放式吸痰方法，B组采用密闭式吸痰方法。分别记录两组吸痰前、吸痰结束即刻、吸痰后1min、3min的收缩压（SBP）、舒张压（DBP）、心率（HR），血氧饱和度（SPO2）变化，并计算SBP和HR的乘积（RPP）。结果A组吸痰结束即刻HR、SBP、DBP、RPP，B组吸痰结束即刻SBP、RPP与吸痰前比较显著升高（P〈0．05或〈0．01），两组吸痰结束即刻SFQ与吸痰前比较显著降低（P〈0．05），A组吸痰后3min、B组吸痰后1、3min与吸痰前比较SPQ显著升高（P〈0．05或〈O．01）。两组吸痰前和吸痰结束即刻各指标比较无显著性差异（P〉0．05）。B组SP02在吸痰后1、3min与A组比较显著增高（P〈0．01），吸痰后1min RPP与A组比较显著降低（P〈0．05）。结论密闭式吸痰法可有效的维持ICU机械通气患者血流动力学的稳定，减轻低氧血症，同时降低心肌耗氧量。

Comparative Study on Different Tracheal Methods Applied in Patients during Mechanical Ventilations in ICU
