NPM和TACC1在胃癌组织中的表达及其临床意义

摘    要:目的 探讨核磷蛋白(NPM)和转录相关酸性卷曲蛋白1(TACC1)在胃癌组织中的表达,并分析其表达与胃癌临床病理特征及预后的关系。方法 采用免疫组化法检测130例胃癌组织及对应癌旁组织中NPM和TACC1的表达,分析两者表达与临床病理特征及预后的关系。采用Kaplan-Meier法进行生存分析,采用Cox回归模型分析影响胃癌预后的独立因素。结果 胃癌组织中NPM和TACC1的高表达率分别为65.4%(85/130)和48.5%(63/130),均高于癌旁组织的51.5%(67/130)和32.1%(42/130),差异均有统计学意义(P<0.05);NPM表达与肿瘤分化程度、TNM分期和复发转移有关,TACC1表达与肿瘤TNM分期有关(P<0.05);NPM和TACC1在胃癌组织中的高表达率呈正相关(r=0.220,P<0.05)。NPM和TACC1高表达者的中位生存期(OS)分别为20.0个月和18.0个月,低于低表达者的32.0个月和22.0个月,差异均有统计学意义(P<0.05)。多因素生存分析显示,NPM和TACC1高表达为影响胃癌患者预后的独立因素(P<0.05)。结论 NPM和TACC1可能参与胃癌的发生、发展,可考虑将两者作为评估胃癌临床肿瘤生物学行为及预后的指标。

Abstract:Objective To explore the expressions of nueleophosmin(NPM) and transforming acidic coiled-coil proteins(TACC1) in gastric carcinoma, and analyze their correlation with clinicopathological features and prognosis. MethodsThe immunohistochemistry was used to detect the expressions of NPM and TACC1 on tissue sections from 130 gastric carcinoma and paired adjacent tissues. Then the relationships between expressions of NPM and TACC1 and clinicopathological variables(age, gender, degree of differentiation, HER-2 status, TNM stage, recurrence and metastasis were analyzed. The overall survival of patients was estimated using the Kaplan Meier method. The factors affecting the prognosis of gastric cancer were analyzed by Cox multivariate survival analysis. Results The high-level expression rates of NPM and TACC1 were 65.4%(85/130) and 48.5%(63/130), higher than 51.5%(67/130) and 32.1%(42/130) of paired adjacent tissues with significant differences(P<0.05). The NPM expression was related with degree of differentiation, TNM stage, recurrence and metastasis, and the TACC1 expression was related with TNM stage(P<005). Positive correlation was observed between expressions of NPM and TACC1(r=0.220, P<0.05). The median overall survival of patients with NPM and TACC1 high level expression was 20.0 and 18.0 months, lower than 32.0 and 22.0 months of patients with lower level expression. Multivariate survival analysis showed that NPM and TACC1 expressions may act as independent prognostic factors for gastric cancer(P<0.05). Conclusion Expressions of NPM and TACC1 may be involved in the occurrence and development of gastric carcinoma. They may serve as markers for evaluating the tumor biological behavior and predicting prognosis of gastric cancer patients.
