深呼吸法与间断无创通气治疗老年慢性阻塞性肺疾病

摘    要:目的探讨提高慢性阻塞性肺疾病(COPD)者呼吸功能的治疗方法。方法5例PaCO260~70mmHg者间断无创通气治疗,1.5h/次,2~3次/d。4例PaCO250~60mmHg者接受间断深呼吸和呼吸方式训练,首先纠正胸腹矛盾运动,每次深呼吸10~20次,3~5次/d。结果两组治疗后血气指标达到或接近正常。活动能力明显增加,无创通气组平均住院日为治疗前的18.64%,平均住院费为治疗前的12.72%。结论间断无创通气和深呼吸法均为较好的提高COPD患者呼吸功能的方法。

Abstract:Objective To investigate the ways of improving respiratory function of patients with chronic obstuctive pulmonary disease(COPD) .Methods Discontinous non-invasive positive pressure ventilation(DNIHV)was used in 5 patients whose level of PaCO2 was 60~70 mmHg(1.5 hourseach time, 2~3 times/day). 4 patients whose level of PaCO0 was 50~60 mmHg accepted discontinuos deep breath and respiratory pattern traninig(deepbreath: 10~20 times each time, 3~5 times/day). Results All patients ' level of arterial blood gas reached or approached normal levels after treat-ment Exercise capacility improved significantly. Mean hospitalization phase decreased significantly (117 days vs 9 days for one year), and mean costdecreased by 12. 72% in DNIHV group. Conclusion Both DNIHV and deep breath could improved respiratory function of patients with COPD.
