原发性肝癌合并动静脉瘘的血管内介入治疗

目的探讨采用不同粗细的高压消毒明胶海绵颗粒及无水酒精治疗原发性肝癌伴动静脉瘘的疗效。方法本组31例原发性肝癌合并动静脉瘿（AVS）行超选择性瘘口栓塞，根据瘘口闭塞程度、肿瘤染色情况确定是否再行肿瘤碘化油栓塞治疗。结果31例中24例一次性瘘口闭塞成功，其中1例因瘘口较大采用弹簧圈加明胶海绵粗颗粒闭塞瘘口成功。31例中26例成功地行肿瘤碘化油栓塞治疗。结论根据原发性肝癌伴动静脉疼（AVS）瘘口分流量大小，采用不同辁塞材料及方法拴塞瘿口是可行的；高压消毒明胶海绵颗粒及无水酒精对肝动静脉瘘（AVS）痿口栓塞是价廉、安全、有效的。

Objective To investigate the effect of superselective embolization with high-pressured gelatin sponge particles and absolute alcohol at the shunt of arterial-portal venous shunts resulted from primary liver cancer.Methods 31 cases of primary liver cancer complicated by arterial-portal venous shunts（AVS） were generally performed on superselective embolization with gelatin sponge particles and absolute alcohol at the shunt ,if unsuccessfully then were carded out on interventional therapy of embolization with iodized oil at the tumor according to the condition of embolism at the shunt and blood vessel of the tumor.Results 24 cases were successfully performed on embolization ,and gelatin sponge particles and coil were used in leases. 26 of 31 cases successfully were carried out on therapeutic embolization with iodized oil at the tumor. Conclusion According to shunted blood of arterial-portal venous shunts resulted from primary liver cancer, different embolism material and therapy can be used. Superseleetive embolization with high-pressured gelatin sponge particles and absolute alcohol is cheap ,safe and effective to the therapy of arterial-portal venous shunts resulted from primary liver cancer
