腹部手术单次预防性抗生素的应用

正确选择与恰当使用预防性抗生素对预防手术部位感染起着十分有效的作用。近年来，提倡用单次预防性抗生素．其效果如何值得探讨。本文对20例行根治性全胃切除术的贲门癌患者手术中与手术后血液与组织的抗生素浓度进行前瞻性测定，探讨单次预防性应用抗生素的合理性。研究表明，对持续时间长、创伤较大的清洁或可能污染的手术，术前一次应用抗菌谱广、半衰期长的头孢三嗪预防术后感染是合理的；对于创伤小、手术持续时间较短的，庆大霉素等半衰期短的药物仍不失为有效的预防性抗生素。

Single-dose prophylactic antibiotics in abdominal surgery

Proper selection and application of prophylactic antibiotics is very important in the prevention of postoperative infection. 20 patients receiving radical total gastrectomy for cancer of the cardia were chosen for the determination of concentration of antibiotics in the blood and the tissue during and after the surgical procedure. The result indicated that single dose of ceftriaxone was suitable for operations with long duration and major trauma,while gentamycin and other antibiotics with short half life were only indicated in minor surgery.
