中药津力达丸与西药联合应用消除2型糖尿病高血糖毒性的临床观察

目的探讨中药津力达丸与小剂量磺脲类和二甲双胍联合应用,消除2型糖尿病高血糖毒性的有效性。方法选择2型糖尿病96例,空腹血糖(FBG)>8.0mmolL,餐后2h血糖(2hPG)>11.0mmolL,糖化血红蛋白(HbA1c)>7.5%,持续1个月以上者。开放性研究,治疗前后自身对照。观察本方案对FBG,2hPG,HbA1c,空腹及餐后1h、2h、3h血浆胰岛素水平,治疗前后血脂的变化。结果治疗6个月后FBG、2hPG、HbA1c较治疗前显著下降,治疗前后比较有显著性差异(P<0.01)。甘油三酯(TG)水平下降,高密度脂蛋白(HDL)升高,较治疗前有显著性差异(P<0.01)。胆固醇下降不明显(P>0.05)。空腹胰岛素(FINS),餐后1h、2h血浆胰岛素(PINS1h、PINS2h)水平显著升高(P<0.01),餐后3h血浆胰岛素(PINS3h)无显著性差异(P>0.05),达到餐时血糖调节作用。结论中西药联合应用在解除高血糖毒性的基础上,HbAlc及血脂也得到了改善,能有效纠正胰岛素早期快速分泌峰缺失,起到了保护胰岛β细胞功能的作用。此外,中药能调整机体脏腑功能,增强体质,减少西药副作用,标本同治,患者依从性强,无低血糖发生,未出现肝肾功能损害。

Clinical observation of Chinese herb Jinlida pill combined with western medicine on elimination hyperglycemia toxicity of type 2 diabetes mellitus

Objective To explore the effectiveness of Chinese herb Jinlida pill on elimination hyperglycemia toxicity of type 2 diabetes mellitus. Methods 96 patients with type 2 diabetes mellitus, fasting plasma glucose (FPG) >8.0 mmol/L, two-hour plasma glucose (2h PG)>11.0 mmol/L, hemoglobinA1c (HbAlc)>7.5%, and lasted for one month, were registered into the experiment. The change of FBG, 2h PG, Hb1c, and fasting, 1-hour, 2-hour, 3-hour post-prandial insulin level were investigated before and after treatment. The level of blood fat was also observed. Results After six months treatment, the level of FPG, 2h PG, Hb1c, and the triglyceride level significantly decreased (P<0.01). The level of high density lipoprotein significantly increased (P<0.01). There was no significant difference in the level of cholesterol before and after treatment (P>0.05). The fasting, 1-hour, 2-hour, 3-hour post-prandial insulin level significantly dropped (P<0.05), while there was no significant difference in the level of 3-hour post-prandial insulin (P>0.05). Conclusion Combined Chinese medicine with western medicine can eliminate hyperglycemia toxicity of type 2 diabetes mellitus, and also can effectively correct the absence of insulin secrete apex in the early stage, thus protect the function of B islet cells.
