切开复位内固定治疗AO-C型踝关节骨折

目的探讨AO-C型(以下简称C型)踝关节骨折治疗的手术方法及时机。方法对21例C型踝关节骨折行切开复位内固定,平均随访20个月。其中,C1型7例,C2型12例,C3型2例。3例Gustilo II型开放骨折I期清创缝合钢板螺钉内固定。其余首先跟骨牵引,待肿胀消退后再行手术。手术方法:腓骨骨折(19例)均用钢板坚强固定。胫骨骨折:三叶草钢板固定19例(其中4例置于前侧),普通钢板2例。5例行I期外侧全厚皮片移植。结果按Baid和Jackson踝关节评分系统评分:优7例,良10例,可3例,差1例,优良率81%。并发症10%。结论切开复位内固定治疗AO-C型踝关节骨折可取得较好疗效。

Treatment of AO-C ankle fracture by ORIF

Aim To investigate the treatment and timing operation of AO-C ankle fracture.Methods Twenty-one cases of AO-C ankle fracture were treated with ORIF and followed up,the average period of follow-up was 20 months.There were 7 cases of type C1,12 cases of type C2,2 cases of type C3 fracture land.Three cases of gustilo II open fracture had been treated by one stage irrigation and debridement,reduction and plate fixation,closure wounds.The other was initially treated by calcaneal traction followed by ORIF as soon as the soft tissues had healed and swelling started to subside.Results According to Baird and Jackson ankle scoring system,the results of treatment were evaluated as excellent in 7 cases,good in 10 cases,fair in 3 cases,poor in 1 cases.The excellent and good rate was 81%.The total percent of complications were 10%.Conclusion ORIF may acquired a better result in treatment of AO-C ankle fracture.
