阿芬太尼复合依托咪酯–丙泊酚混合液在老年患者无痛胃镜中的应用

目的 观察阿芬太尼复合依托咪酯–丙泊酚(EP)混合液在老年患者胃镜检查中的麻醉效果及术后恢复质量的影响。方法 选取2021年4月至2021年9月在嘉兴市第二医院内镜中心行无痛胃镜检查的223例老年患者作为研究对象，采用随机数字表法分为阿芬太尼复合EP混合液组(观察组，n=114)和EP混合液组(对照组，n=109)。记录诱导前(T0)、入镜前(T1)、胃镜过咽喉部(T2)、检查结束时(T3)、苏醒时(T4)以及苏醒后30min(T5)，两组患者的麻醉心率、平均动脉压和氧饱和度。比较两组的胃镜操作时间、苏醒时间、定向力恢复时间、EP混合液用量、内镜医师满意度、患者满意度以及围术期不良反应等。结果 T1～T3时，观察组HR显著高于对照组(P<0.05);T1～T4时观察组MAP显著高于对照组(P<0.05);T1时观察组SpO₂显著高于对照组(P<0.05)。观察组EP混合液用量显著少于对照组(P<0.05)，苏醒时间及定向力恢复时间显著短于对照组(P<0.05)。观察组的内镜医生满意度评分显著高于对照组(P<0.05)，低氧血症、...

Application of alfentanil compound etimidate-propofol mixture in painless gastroscopy in elderly patients

Objective To observe the effect of low-dose alfentanil compound etomidate-propofol (EP) mixture in gastroscopy and the quality of postoperative recovery in elderly patients. Methods A total of 223 elderly patients underwent painless gastroscopy in the endoscopy center of the Second Hospital of Jiaxing from April 2021 to September 2021 were divided into alfentanil compound EP mixture group (observation group, n=114) and EP mixture group (control group, n=109) according to random number table method. Heart rate, mean arterial pressure and oxygen saturation were recorded before anesthesia induction (T0), before endoscopy (T1), after endoscopy (T2), at the end of examination (T3), at the time of awakening (T4) and 30 min after awakening (T5). The operation time, recovery time, orientation recovery time, EP mixture dosage, endoscopist satisfaction, patient satisfaction and perioperative adverse reactions were compared between the two groups. Results From T1 to T3, HR was significantly higher in the observation group than that in the control group (P<0.05). From T1 to T4, MAP was significantly higher in the observation group than that in the control group (P<0.05). At T1, SpO2 was significantly higher in the observation group than that in the control group (P<0.05). The EP mixture dosage used in the observation group was significantly less than that in the control group (P<0.05), and the recovery time and orientation recovery time were significantly shorter than those in the control group (P<0.05). The endoscopist satisfaction score was significantly higher in the observation group than that in the control group (P<0.05), and the incidence of hypoxaemia, hypotension, body movement, dizziness and nausea and vomiting were significantly lower than those in the control group (P<0.05). There was no statistically significant difference between the two groups in terms of operation time, patient satisfaction scores, and incidence of choking, erratic cough and myalgias (P>0.05). Conclusion Low-dose alfentanil compound EP mixture can make the circulation and respiratory function of elderly patients more stable during gastroscopy, less perioperative adverse reactions and higher postoperative recovery quality and it is a good choice for elderly patients undergoing gastroscopy.
