系统性红斑狼疮外周血单个核细胞周期相改变及药物影响

摘    要:【目的】研究系统性红斑狼疮(systemic lupus erythematosus SLE)外周血单个核细胞(peripheral bloodmononuclear cells PBMCs)的细胞周期相改变和药物的影响,探讨其在SLE发病中的作用。【方法】用流式细胞分析技术,检测SLE患者(活动期和静止期)PBMCs的细胞时相构成比例,观察药物对其的影响,并和正常人进行比较;分析SLE患者细胞周期时相构成比例和血清抗dsDNA抗体的相关性.【结果】①基础状况下,SLE患者PBM-Cs的细胞周期时相构成比例(%)和正常人之间无明显差别(均P>0.05)。②植物凝集素(phytohemagglutinin,PHA)使活动期SLE患者PBMCs的G_0/G_1期比例减低(P<0.01)和S期比例增高(P<0.01)、缓解期SLE患者PBMCs S期比例增高(P<0.01)、正常人PBMCs各时相均无明显改变。③用地塞米松(dexamethasone,Dex)后,活动期SLE患者PBMCs各期时相比例无改变,缓解期SLE患者PBMCs G_0/G_1期比例增高(P<0.001)和S期比例减低(P<0.001),正常人PBMCs的G_0/G_1期比例增高(P<0.05)。④活动期SLE患者血清抗dsDNA抗体和G_0/G_1期比例呈负相关(r=-0.787,P=0.001)、和S期比例呈正相关(r=0.855,P=0.0001)。【结论】SLE患者PBMCs具有较强的潜在增殖活性,其时相构成比例和血清抗dsDNA抗体相关,可能和其发病及病程有关。

Abstract: Objective ] To investigate the changes of cell cycle-phase of peripheral blood mononu-clear cells (PBMCs) in patients with systemic lupus erythematosus (SLE) and the effects of all changes by drugs. Methods] The constituent ratio of different cell cycle-phase of PBMCs in patients with active and inactive SLE and health control were examined by flow cytometric analysis. After stimulated by phytohemagglutinin (PHA) and depressed by dexamethasone (Dex), these examinations were repeated. The correlation between constituent ratio of different cell cycle-phrase of PBMCs and serum anti-dsDNA antibody level were analysed. Results] (1) There were no significant difference at the constituent ratio of different cell cycle-phase of PBMCs in SLE and health control at basal status. (2) Stimulated by PHA, the constituent ratio of phase Go/G1 of PBMCs decreased markedly in active SLE ( P < 0. 01), the phase S increased in active and inactive SLE (all P < 0. 01), and there were no changes of PBMCs in health control. (3) Depressed by Dex, the constituent ratio of phase Go/Gi of PBMCs increased significant in inactive SLE ( P < 0. 001) and health control ( P < 0. 05), the phase S decreased in inactive SLE (P < 0. 001), and there were no changes of PBMCs in active SLE. (4) In patients with active SLE, there were negative correlation between anti-dsDNA antibody level and constituent ratio of phase G0/G1( P = 0. 001), there were positive correlation between an-ti-dsDNA antibody level and constituent ratio of phase S( P = 0.0001). Conclusion] There were strong latent proliferation activity of PBMCs in patients with SLE and correlation between constituent ratio of different cell cycle-phrase of PBMCs and serum anti-dsDNA antibody level. These may be related to the pathaeenesis etiology and the course of SLE.
