沙美特罗替卡松粉吸入剂对初入高海拔地区青年抗低氧效果的观察

摘    要:目的观察沙美特罗替卡松粉吸入剂对初入高海拔地区青年的抗低氧效果。方法将34名受试者随机分为沙美特罗组(n=18)和对照组(n=16),自海拔1 400 m历时5 d进入5 200 m,从进入海拔5 200 m当天开始,沙美特罗组每天早晚各1次吸入沙美特罗替卡松粉准纳器1个剂量(沙美特罗100μg,丙酸氟替卡松200μg),连续5 d;对照组用同样的方法吸入少量生理盐水。以军用卫生标准GJB1098-91《急性高原反应的诊断和处理原则》随访观察第1、3、5 d急性高原反应的严重程度、血氧饱和度(SaO2)和心率(HR)。结果进入海拔5 200 m第1、3、5天沙美特罗组较对照组急性高原反应分值均降低,差异有非常显著统计学意义(P<0.01或P<0.001);SaO2均增高,差异有显著统计学意义(P<0.05);HR虽有降低,但无统计学意义(P>0.05)。结论吸入沙美特罗替卡松粉吸入剂可改善高原低氧血症,降低急性高原反应的发病率。

Abstract:Objective To explore the effect of salmeterol inhalation on the anti-hypoxia of the transients at high altitude.Methods Thirty-four subjects reaching the high altitude of 5,200 meters(chronometer spot) from an altitude of 1,400 meters were divided into two groups,i.e.trial group(n=18) and control group(n=16).The trial group inhaled one dose of salmeterol powder(100 μg salmeterol and 200 μg fluticasone propionate) and the control group inhaled placebo(0.9% sodium chloride) twice daily for five days.The altitude reactions were measured with the Acute Mountain Reaction Diagnosis and Treatment Principle on the 1st,3rd and 5th day when they reached the altitude of 5,200 meters.Results Compared with that in the control group,acute mountain reaction scores reduced and arterial oxygen saturation(SaO2) increased significantly in the trial group on the 1st,3rd and 5th day(P<0.01,or P<0.001,or P<0.05).There was no significant difference in heart rate(HR)(P>0.05).Conclusion Salmeterol can improve hypoxemia and reduce acute mountain reaction at high altitude.
